How do I make an appointment?

When you phone for an appointment with a doctor (0117-955-7474) you will be asked for brief details of the problem and a contact telephone number;

  • The doctor will then phone you back as soon as they can to discuss your problem with you;
  • If you can only receive calls at certain times (e.g. work breaks etc.) please tell the receptionist and they will ask the doctor to ring during those times;
  • Often, you will not need to see the doctor in person and the problem can be dealt with fully over the phone;
  • If the doctor does need to see you he/she will give you a time to come to the surgery to see them.
  • If you are housebound the doctor may be able to visit you at home. They will arrange this with you when they phone you.

To see anyone OTHER than a doctor, please phone our reception desk (0117-955-7474) and one of our receptionists will be happy to book you an appointment on a day and time to suit your needs.

 

How do I get a repeat prescription?

Wherever possible, repeat prescriptions should be ordered using the right hand side from your previous prescription. If you do not have this we have a form at our reception desk which you can use to request the medicines you need.

Your request can be handed in at the surgery reception desk or posted to us.

At some local pharmacies you can hand in your request and they will bring it to the surgery and collect the prescription when it is ready. Please ask your local pharmacy for more details.

Please try to order all the medication you need for the next month together.

What do I do in an emergency?

A serious medical emergency is something that, if not dealt with quickly, may rapidly become life threatening.

If you have what you consider to be a serious medical emergency please call the AMBULANCE SERVICE by dialling 999.

 

How do I get my test results?

Test results can be given over the phone, but for confidentiality reasons, only to the person who had the test. Please call the surgery after 2pm to get you result, as test results arrive around lunchtime.

How long it takes for a test result to arrive will vary from test to test. If possible, ask the Doctor / Nurse who has requested the test how long the result should take to come. Otherwise, you can contact reception on 0117 955 7474 and they will be able to find out for you.

 

How do I register with the practice?

To register with the Wellspring Surgery, you will need to complete our registration form. You can pick up a copy from the surgery reception desk. We need to know your contact details and some details about your health. To help us get your medical records as quickly as possible, we need to know who your previous doctor was.

When you return your completed form to us we will need you to fill in an NHS Registration form (GMS1)
We will also two forms of proof of identity

One that is picture ID:

  • Current driving licence;
  • Current passport;

And one that gives proof of current local address:

  • Home Office Documents;
  • Utility bill/bank statement (in your name and dated in the last 3 months);
  • NHS medical card or EU medical card;
  • Benefits Office letter to home address (dated within the last 3 months);
  • Old style NHS medical card with NHS number;
